Patient Participation Group

We are encouraging patients to give their views about how the practice is doing. We would like to be able to find out the opinions of as many patients as possible.

If you would like to participate you will need to complete our registration form which reception has and return it.

The form will go to the management team who will add your email address to a contact list.

You can be involved via email and/or attend the quarterly meetings.

Any patient who wants to be involved in the quarterly face to face meetings will be contacted in advance and advised of the date and time (time will usually be 12.00pm for 1 hour usually a Wednesday)

Your contact details will only be used for this purpose and will be kept safe. If you decide you no longer wish to be on our contact list, let us know by email / telephone or in person and we will remove you from the contact list.

The Aims of the Patient Participation Group

  • To offer opinions in a constructive manner and to put forward ideas on behalf of other patients.
  • To improve the provision of health care.
  • To improve communication between surgery, patients and the wider community about matters concerning the surgery and health in general.
  • To provide assistance in development of new services.
  • To encourage a spirit of self help and support amongst patients to improve their health and social care.

Virtual Patient Representation Group

We would like to know how we can improve our service to you and how you perceive our surgery and staff.

To help us with this, we are setting up a virtual patient representation group so that you can have your say. We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.

We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.

You can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.

Many thanks for your assistance
