with Patient Access, it’s quick and it’s easy.

All you need to do is complete the Online Access Registration Form and bring it in to the Surgery with some photo ID. You will be given a print off with your ID details and how to register online for the service. Application Forms will not be considered for patients who are under the age of 16.
